In order to schedule a consultation appointment, we'll need a referral from your dental home aka: your regular dentist.

Instructions will be sent to you to fill out your child's new patient packet online before your appointment. Filling out this packet is required in order for us to keep your appointment.
New Patient
Paperwork:
Arrive to our office for your consultation appointment. A legal guardian must accompany your child. Here we will examine your child, provide a tentative treatment plan, and discuss all pre-surgery instructions. Surgery will not occur at this appointment.
Consultation:

We'll provide health and physical paperwork for your pediatrician. They'll assess your child's fitness for anesthesia to make sure we're proceeding in the safest way possible.
Visit your pediatrician:
Confirm Surgery Appointment:
After receiving your pediatrician's report, we'll contact you to confirm your surgery appointment.
Arrive at the surgery center - a legal guardian must accompany your child. Be sure to review and follow all pre-surgery instructions to avoid procedure postponement.
Surgery Day:
Following surgery, return back to your regular dentist for continued cleanings and check-ups
Return to Dental Home:
It's important to know we are not a regular dental office or "dental home" that you see every 6 months. We provide comprehensive dental treatment under general anesthesia based on a referral from your dental home. After treatment is completed, we will refer you back to your dental home for continued care.
We must receive a referral from your dental home in order for you to qualify for care with us.
